What's the fastest way to get a grill up to temperature?

Set up: Chimney starter, lump charcoal, kettle grill

Once you pour the coals in, what gets the grill up to its desired temperature fastest? Should you leave the cover off (maximizing oxygen) or leave the cover on with the vents open? Thank you in advance.
